Once you have reached out and confirmed an appointment time that works best, you will be sent a welcome email containing all intake forms pertaining to your appointment type. The welcome email will also include your date and time, a breakdown of what you can expect during your first appointment, directions and parking instructions.
We ask that you fill out your forms ahead of time and bring them with you to your first appointment.
New Patient Forms

- Naturopathic Intake and Case History Form
- Declaration and Consent of Treatment Form
- Authorization of Release Form
Try and arrive early to avoid losing time with your practitioner!
If you have not had time to fill out your forms ahead, we ask that you arrive 30 mins early.
Arriving early ensures that the time reserved for your appointment/treatment will not be used up while you fill out your forms
