PCOS doesn't arrive with a simple explanation. It shows up as a collection of frustrating, sometimes confusing symptoms — and often, a collection of frustrating, sometimes dismissive answers from the doctors you've already seen.
Dr. Fiona has lived this. Her own PCOS symptoms began at age 12 — severe acne, cycles that came twice or three times a year at most, and years of treatments that never addressed the root cause. She wasn't diagnosed until her late 20s. That experience shaped everything about how she practices today.
Which symptoms match your experience?
These patterns can help you recognize which type of PCOS may be contributing to your symptoms. Most women have features of more than one type — that's common.
This table is for general orientation only. PCOS types can be confirmed through lab testing and clinical assessment — many women have mixed presentations or factors impacting their PCOS. Some patterns that resemble PCOS are driven by different underlying mechanisms (thyroid dysfunction, elevated prolactin, or other hormonal patterns). Clarifying the diagnosis is part of our initial assessment.
Common experiences with PCOS
PCOS presents differently depending on your type, and factors involved. Much of the data around understanding types is still in research. Many women recognize themselves in one or more of these areas commonly impacting PCOS. Some symptoms may be more or less predominant in each type.
Insulin-resistant PCOS
"Lean" PCOS (non-insulin resistant)
Androgenic Symptoms of PCOS
PCOS: Stress, Mood and Brain
Fertility Concerns in PCOS
These experiences can overlap, and they can have many contributing factors. A thorough assessment helps clarify the pattern and guide next steps.
If you have insulin-resistant PCOS: difficulty losing weight despite consistent diet and exercise is a predictable physiological consequence of insulin resistance and blood sugar dysregulation — not a personal failing. Addressing insulin response, not restricting calories, is typically what makes the difference.
What often makes it harder
Why Ontario Women with PCOS Choose White Lotus Clinic
Clinical philosophy
Many women with PCOS have been treated with a one-size-fits-all approach — the same supplements, the same diet advice, the same blood tests. That doesn't work because PCOS isn't one condition. It presents as several distinct types, each driven by different underlying factors, and each requiring a different treatment strategy.
Dr. Fiona's approach begins with identifying your specific PCOS type and factors requiring treatment in your case. Treatment that isn't matched to your type or individual needs is treatment that's likely to disappoint. As she describes it: "I always listen very carefully and extensively to their whole life story with their health, and also what they really hope for. I always try to customize the treatment so it's something they can do in their daily life, matches their personality type or their goals."
But identifying the right type and factors to treat is only part of it. The other half is calibrating the appropriate level, timing and duration of intervention. Severe insulin resistance — one of the most common and most undertreated patterns in PCOS — doesn't respond to a basic supplement. It requires an intensive, multi-pronged protocol sustained over months. A different presentation may not respond to insulin-sensitizing treatment at all, and needs a different approach entirely. Knowing which is which, and committing to the right intervention for long enough, is where most prior treatments have fallen short.
Many patients describe their initial consultation as the first clinical conversation where someone asked about their whole history — not just their most recent lab result.
